Complete the Credit Card Authorization Form and send back via fax (+1 203 542 0661) with:
- A copy of the picture ID of the credit card holder (credit card holder has to be from the same country as the applicant)
- A copy of the credit card, front and back
- Check/Money Order - Make checks/money orders payable to “Friendship Ambassadors Foundation, Inc.”
